Integrated Health Service (IHS)is a program that actively involves the entire family through a team of specialists, along with your current service providers, to provide resources, support, and referrals and ensure you have the right care for your child, including medical, mental and physical health, help with food, other social support, and advocacy in school and court settings. For children up to the age of 18.

IHS is a person-centered program that focuses on the goals of the member and their family. As part of IHS, we assist families in all social determinants of health (SDoH) areas, Economic Stability (employment and unemployment resources, assistance with disability determinations), Education Access and Quality (we know the system, provide resources and advocacy, attend meetings with families), Health Care Access and Quality (access to a registered nurse, referrals and assistance in locating providers, medication education, new diagnosis education), Neighborhood and Built Environment (assistance connecting with their community, help them assess and modify their physical environment, housing and utility resources), and Social & Community Context (building their support system, providing activities and opportunities for family engagement, linkage to support groups and NAMI, crisis planning and wellness recovery planning).

Each member/family is assigned a team. Our Resource Specialists are dedicated human service experts who provide overall care coordination between providers and direct the progression of the family's wellness goals. Our Family and Peer Support Specialists offer real experience navigating the health systems as a parent or individual. They offer empathy, understanding, and advocacy to members by assisting them with applications, attending meetings or hearings, and offering non-judgmental open support. Every family has access to a nurse who provides ongoing health information, follow-up on urgent care visits and hospitalizations, participation in transition planning, and medication education.
