If you have been banned from the Council Bluffs Public Library and would like to appeal the decision, you must complete this form.
This form must be complete for appeal consideration.
A banned individual may request an appeal of the ban using this form within ten working days of the date of the ban notification. Until you receive notification of your ban repeal, you are still banned from the library. (See Policy 3.11 Ban and Appeal)
